If you have had an illness, accident, just come home from hospital or are becoming frail, you might find it difficult to look after yourself. You might not feel strong enough to cope or you may simply have lost confidence.
The aim of our service is to help people regain the ability to perform their usual activities: like cooking meals, washing and getting about, so that they can do things for themselves again.
Generally, our service is for people aged 18 and over, who need support through Reablement and may have one of the following needs:
- Be frail or have limited mobility;
- Have a physical disability;
- Have a sensory impairment;
- Have a memory impairment;
- Have dementia;
- Have learning disabilities;
- Have mental health needs; and
- Have had an acute medical illness or accident.
Our approach is person-centred which is about promoting and maximising independence to allow people to remain in their own home as long as possible. Our short period of intensive support in your home is therefore designed to enable you gain or regain your confidence, ability, and necessary skills to continue living independently, especially after having experienced a health or social care crisis, such as illness, a deterioration in health or injury.
We will work with you and focus on your strengths & abilities to help you regain old skills, or develop new ones. In most cases, this is all you need to regain your independence and be able to carry on living in your own home. We find that many people just need a bit of help to get back on their feet.

The support we provide
We can be flexible around the time you wake up, meal times and any other activities you may have.
Your support plan will be designed around your individual needs. We will work with you to draw up plans for the care, therapy and support you may need. We will work with you and your family to agree how to gradually reduce your support as you are regaining your daily living skills, improving your quality of life and your confidence.
Our Team Leader will talk to you about:
- What you want to be able to do in the future;
- What goals you need to work on, for example getting in and out of the bath, preparing meals, walking to the shops or going back to work; and
- How they are going to help you with those goals.
Our support staff will work with you to support you to regain skills with various practical tasks including:
- Washing, bathing, dressing;
- Cleaning and laundry;
- Using the toilet;
- Taking your medication;
- Shopping;
- Eating and drinking;
- Meal preparation;
- Getting up or going to bed; and
- Restoring mobility and building your confidence (where required with the use/help of appropriate equipment and Telecare).
During the service you will have a Reablement Action Plan that clearly sets out what support is required, your goals and for how long.
